Client Waiver

Effective Date: January 1, 2025 1. Introduction: By booking and attending a coaching session, you agree to abide by the following terms and conditions. Please read them carefully. 2. Definitions: Client: The individual receiving a coaching session whose name is stated above. Coach: The individual providing coaching services - Bryony Wright of Slow Burn Wellness. Appointment: A scheduled session between the Client and the Coach. 1. Purpose of Coaching: I understand that integrative wellness coaching is a collaborative process designed to support personal growth, wellness, and fulfillment. Coaching does not include medical diagnosis, treatment, or therapy, and the Coach is not a licensed healthcare professional. 2. Client Responsibility: I understand that I am responsible for my own health and well-being throughout the coaching process. I will consult a licensed medical professional for any medical concerns. I acknowledge that coaching outcomes are dependent on my commitment and effort. 3. Confidentiality: All information I share during sessions will remain confidential unless: a) Required by law (e.g., risk of harm to self/others). b) Explicit written consent is provided by me. 4. Assumption of Risk: I acknowledge that wellness coaching may involve discussing sensitive topics that could evoke emotional responses. I voluntarily assume all risks associated with the coaching process, including any impact on my physical, emotional, or mental well-being. 5. Limitation of Liability: I release and hold harmless the Coach and their business from any and all claims, liabilities, or damages, including but not limited to injury, loss, or harm arising directly or indirectly from coaching sessions. 6. No Guarantee of Results: I acknowledge that the Coach does not guarantee specific results or outcomes. Success depends on my active participation and efforts. 7. Cancellation Policy: I agree to notify the Coach at least 24 hours in advance if I need to cancel or reschedule an appointment. Cancellations or changes made with less than 24 hours' notice may be charged in full. 8. Governing Law: This agreement is governed by the laws of the Province of British Columbia and the applicable federal laws of Canada. 9. Acknowledgment and Consent: By checking the box below, I confirm that: I have read and understood this waiver. I agree to the terms and conditions voluntarily. I understand that this agreement is legally binding.
